We provide a variety of hospice care services to fit the unique needs of our patients and their families.

Nursing Services

  • Our nurses provide care where the hospice patient resides (private home, nursing home, assisted living, etc.)
  • Each patient is assigned an RN Case Manager to coordinate patient care
  • In coordination with other hospice team members, your nurse develops a person-centered plan of care
  • Our nurses are available to you 24 hours-a-day, 7 days-a-week
  • Nursing visits are based on patient-specific plan of care but at least once per week (RN or LPN)
  • Assessment and management of pain or other symptoms
  • Provide education on end of life issues
  • Provide emotional and spiritual support for end of life issues
  • Assistance with medications, medical equipment, and supplies

Therapy Services

  • Physical Therapy/Occupational Therapy/Speech Language Pathology Therapy
  • Therapy visits are based on patient-specific plan of care
  • Performance of patient and/or home safety assessment
  • Provide training in the use of adaptive equipment or assistive devices
  • Develop compensatory strategies to maintain quality of life

Hospice Aide and Homemaker Services

  • Hospice Aide visits are based on patient-specific plan of care
  • Provide assistance with personal care (bathing, dressing, grooming, etc.)
  • Provide assistance with light housekeeping
  • Provide emotional support for end of life issues

Physician Care

  • Our Hospice physician provides medical oversight of patient care collaborating with other Hospice team members
  • Actively involved in developing a patient-specific plan of care
  • Collaborates with patient’s primary physician
  • Prescribes appropriate medications
  • Provide education on end of life issues
  • Provide emotional and spiritual support for end of life issues

Medical Social Worker Services

  • Medical Social Worker visits are based on patient-specific plan of care (typically 1-2 times a month)
  • Actively involved in developing a patient-specific plan of care
  • Performs a psychosocial assessment to determine any needs of patient
  • Provides anticipatory grief support to patient and family
  • Provides assistance with Advance Care Planning and Mortuary planning
  • Provides assistance with obtaining needed community resources
  • Provide education on end of life issues to patient and family
  • Provide emotional support and counseling for end of life issues

Spiritual Care

  • Spiritual care visits are based on patient-specific plan of care (typically 1-2 times a month)
  • Provides non-denominational spiritual support to patient and family
  • Collaborates with patients identified spiritual clergy

Volunteer Services

  • Visits are based on patient-specific plan of care
  • Volunteers are hospice trained
  • Provide emotional support
  • Provide companionship/conversation
  • Provide Vigil support (presence during the patients final hours)
  • Provide assistance with errands
  • Provide short respite break assistance for family
  • Make check-in phone calls

Bereavement Services

  • Bereavement visits are based on patient-specific plan of care
  • Provide grief support associated with a decline in health status and the dying process
  • Available before and after loss of loved one
  • Available to family up to 13 months after the loss of a loved one
